Employer's First Report of Injury or Illness (Note: Use the Internet Explorer browser when completing this report)

Supervisor's Accident Investigation Report (Complete and submit to Public Safety in addition to Employer's First Report)

Worker's Compensation Employee Notification Form (Print and have employee read and sign. Send copy to Public Safety)

List of Panel Physicians (Employees with a work related injury that need advanced aid must see a panel physycian)
