National Vision Administrators

This program is offered by National Vision Administrators (NVA). NVA provides its members with significant out-of-pocket savings on eyewear and professional services from an extensive national network of providers. Under this plan, you must utilize a NVA participating provider in order to obtain vision care discounts. There are no forms to fill out! Simply go to a participating provider (throughout the United States) and present your card for the discounted rates on all services and products. The cost of enrolling in this program is $6.60 per family per year. The enrollment form along with the $6.60 premium should be mailed directly to NVA. The NVA card will be printed with the names of all eligible family members and will be mailed directly to your home. Your card will be effective from March 1 through February 28, at which time you will have the option of re- enrolling in the plan. Open enrollment for this plan will be held once a year in February with a March 1st effective date.
