Voluntary Dental Plan

The College offers full-time employees and retirees the opportunity to enroll in a voluntary dental insurance plan offered by United Concordia, a subsidiary of Pennsylvania Blue Shield.

Concordia Flex is a fee-for-service dental plan which offers members the freedom to obtain dental services from any provider of their choice while receiving significant savings. The plan allows participants to obtain dental care both within an established network of dental providers and outside of the network. Unlike some dental plans, Concordia Flex provides coverage for the full range of dental care needs such as routine diagnostic and preventive services, primary, major and orthodontic treatment. The reimbursement percentage for each type of dental service varies by the type of treatment received.

In-Network:

The "Concordia Flex Network" includes a nationwide network of dental providers who participate in the Concordia Flex plan. Sixty percent of the dentists in Pennsylvania participate in the Concordia Flex Network (26 dentists in Easton/194 in Northampton county/194 in Lehigh County). These dentists have agreed to accept United Concordia's schedule of maximum allowances as payment in full for covered services. When you use a participating (in-network) dentist for diagnostic and preventive services, eligible services are co vered in full (100%). When you receive primary, major and/or orthodontic services, you are responsible for paying the corresponding coinsurance amount. (For example, primary services such as an extraction are covered at 80%; therefore, your coinsurance p ayment for these services would be 20%.) When visiting an in-network dentist, the dentist may not charge more than the allowance established by Concordia for the service. Employees utilizing in-network providers will realize significant cost savings on d entists' charges due to Pennsylvania Blue Shield's Participating Provider arrangements.

Out-of-Network:

When utilizing an out-of-network dentist, the plan pays the same fixed percentage of Concordia's set allowance for the particular dental service as it does for participating dentists. The member is then responsible for the copayment plus the differenc e between the dentist's actual charges and Concordia's established allowance for the service.

If you wish to locate a participating dentist or have any questions regarding coverage, United Concordia's customer service department can be reached at 1-800-332-0366 from 8:00 a.m. to 8:00 p.m. You may also access the provi der directory by visiting them on the internet at www.ucci.com.
