Enrollment Information

The College's membership in the Lehigh Valley Business Conference affords our employees the ability to participate in this plan at a very reasonable cost. The monthly rates effective January 1, 2007 are as follows:

Single	$27.94
Two Persons	$55.86
Family	$72.24

Deductions will be made on a pre-tax basis unless you return the "Option for After Tax Dental Insurance Premium" form along with your enrollment form. (If you wish to pay your dental premium on a pre-tax basis through payroll deduction, do not return the "Option" form.) Retirees wishing to participate will be billed quarterly, the same as are those enrolled in the the Lafayette College PPO Plan.

Eligible employees/retirees can enroll at the time of hire and during the yearly open enrollment period (January 1st). If you disenroll from the plan, there is a two year waiting period before you can re-enroll!
